Anterior Perspective of the Anterior Longitudinal Ligament

Running vertically along the ventral surfaces of the vertebral bodies, the anterior longitudinal ligament (ALL) appears as a continuous band spanning multiple spinal levels in an anterior perspective of the male vertebral column. Segmental vertebrae stack superior to inferior with intervertebral discs interposed, while the ligament hugs the anterior margins of the vertebral bodies and crosses the disc spaces with a broader, more adherent attachment over the annulus fibrosus. Within the vertebral canal, the spinal cord is seen posterior to the vertebral bodies, with spinal nerve roots projecting laterally toward the intervertebral foramina. Clear midline orientation makes the anterior, posterior, medial, and lateral relationships unambiguous. Clinically, the ALL is the primary restraint to hyperextension, and this view makes its long, uninterrupted course easier to teach than posterior ligamentous anatomy, which is frequently obscured by laminae and spinous processes. Ossification along the ventral spine, including diffuse idiopathic skeletal hyperostosis (DISH), tracks the ALL distribution and helps explain the characteristic flowing anterior osteophytes and stiffness seen on radiographs.
