Description

Running along the ventral aspect of the vertebral column, the anterior longitudinal ligament forms a broad, dense band applied to the anterior surfaces of adjacent vertebral bodies and the intervening intervertebral discs. In a lumbar segment you would expect to see the ligament widest over the vertebral bodies and more firmly blended with the outer annulus fibrosus at each disc level, with the anterior margins of the vertebral bodies aligned in the sagittal plane. Laterally, fibers taper toward the anterolateral corners of the vertebral bodies, stopping short of the pedicles and transverse processes, while posterior elements such as the spinous processes remain out of view. Functionally, this is the primary restraint to spinal extension and a key stabilizer against anterior shear, so its thickness and attachment pattern matter when teaching lumbar biomechanics. Hyperextension injuries can avulse a bony fragment at the anteroinferior vertebral body margin (an extension teardrop-type mechanism), and degenerative traction osteophytes often form along the ligament’s anterolateral margins where it blends with the disc annulus. An anterior approach to the lumbar spine for discectomy or interbody fusion requires deliberate management of the anterior longitudinal ligament, since incision or preservation changes postoperative stability.
