Description
Paired kidneys lie retroperitoneally on either side of the vertebral column, their smooth, convex lateral borders facing laterally and the medial borders directed toward the spine where the renal hila would be located. From each renal pelvis, a ureter descends inferiorly, tracking along the anterior surface of the psoas major region toward the pelvis. Semi-transparent ribs and the upper pelvis frame the organs, reinforcing the kidneys’ position under the lower rib cage and above the pelvic brim in the adult male. An anterior perspective like this clarifies a common point of confusion in teaching: the kidneys sit posterior to the peritoneal cavity yet project into the upper abdomen, with the left kidney typically positioned slightly more superior than the right because of the liver. That relationship matters when correlating flank pain, renal trauma from lower rib fractures, or planning percutaneous nephrostomy and nephrolithotomy routes that deliberately approach the kidney from a posterior or posterolateral trajectory to stay out of the peritoneal space. Ureteral course also matters. This is where obstruction from a calculus begins its downstream consequences.
