Description
Seen from the anterior in an adult male, the paired kidneys sit retroperitoneally on either side of the lumbar vertebral column, with their superior poles tucked deep to the inferior ribs. Each renal hilum faces medially toward the midline, where the renal pelvis funnels into the proximal ureter that descends inferiorly toward the pelvis. Within the renal sinus, major calyces converge toward the pelvis in a branching pattern that occupies the central, concave portion of the kidney. Skeletal landmarks include the ribcage superiorly and the vertebrae in the midline. An anterior perspective clarifies why the kidneys are relatively protected yet still vulnerable at their lower poles, which are less shielded by ribs and can be injured in blunt flank trauma. The collecting system emphasis, major calyces to renal pelvis to ureter, also matches the anatomic pathway where obstruction produces hydronephrosis, commonly from calculi at the ureteropelvic junction or along the ureter.
