Description
Presented from an anterior perspective, the frontal belly of the occipitofrontalis (epicranius) spans the forehead as paired muscle sheets separated by the midline and blending superiorly into the galea aponeurotica. Inferiorly, its fibers interdigitate with the brow depressor complex, including corrugator supercilii and procerus, and lie superficial to the frontal bone and the frontal sinus region. Surrounding facial expression muscles are also apparent, with orbicularis oculi encircling the palpebral fissures and zygomaticus major and minor coursing inferolaterally from the zygomatic bone toward the modiolus. Cervical landmarks, including sternocleidomastoid running obliquely from mastoid process to manubrium and clavicle, frame the lower face above the upper fibers of pectoralis major. Brow position depends on frontalis tone. This view makes it easier to teach the antagonism between the occipitofrontalis and the brow depressors, a relationship exploited in aesthetic and functional botulinum toxin planning where excessive frontalis weakening can unmask brow ptosis. Temporal branch injury of the facial nerve, often discussed in facelift and temporal approaches, produces frontalis paresis with loss of forehead rhytids and impaired eyebrow elevation. Use this illustration in gross anatomy, head and neck, or facial expression modules to orient learners to layered facial musculature and the continuity between scalp aponeurosis and forehead.
