Description
Centered on the orbital aperture, the orbicularis oculi is highlighted as an oval sphincter of facial expression encircling the palpebral fissure and orbital rim, with fibers spanning the upper and lower eyelids and blending medially toward the medial canthus. Laterally, its fibers sweep toward the lateral palpebral raphe, lying superficial to the bony margins of the frontal bone and zygomatic bone. Surrounding grayscale musculature frames the region, including the frontalis superiorly, the zygomaticus major and minor inferolaterally, and the masseter over the lateral mandibular ramus. A clean anterior perspective emphasizes bilateral symmetry. Functionally, the distinction between the palpebral portion (gentle eyelid closure and blinking) and the orbital portion (forceful closure) is easier to teach from an anterior view where fiber direction around the lid margins is apparent. This is the muscle tested in facial nerve (CN VII) palsy, where incomplete eyelid closure leads to exposure keratopathy, and it is also the target region for botulinum toxin injection in lateral canthal rhytids, where avoiding diffusion toward the levator palpebrae superioris helps prevent iatrogenic ptosis. Small muscle, big consequences.
