Anterior Perspective of the Pectoral Branch of the Thoracoacromial Artery

Arising from the second part of the axillary artery, the thoracoacromial artery pierces the clavipectoral fascia and quickly divides, with the pectoral branch descending inferomedially across the anterior chest wall. From this anterior perspective in an adult male, the vessel tracks deep to pectoralis major and along the superficial surface of pectoralis minor, sending multiple muscular branches that fan medially and inferiorly. Superiorly, the clavicle and anterior shoulder contour frame the proximal course near the deltopectoral interval, where the cephalic vein often runs in close relationship. Small accompanying veins and adjacent arterial pathways toward the axilla and proximal arm can be appreciated in context. This view matters when planning anterior shoulder and chest procedures that traverse the deltopectoral groove or elevate pectoralis major, because injury to the pectoral branch can produce brisk bleeding and postoperative hematoma. It also clarifies collateral circulation in proximal subclavian or axillary occlusive disease, where thoracoacromial contributions can enlarge and anastomose with intercostal and internal thoracic perforators. Clear landmarks. The relationship to the clavipectoral fascia and pectoralis minor is a frequent point of confusion for trainees and shows up repeatedly in operative notes.
