Description
Running deep to pectoralis major near the anterior shoulder, the thoracoacromial artery emerges from the second part of the axillary artery and quickly divides, with the clavicular branch coursing superiorly and medially toward the sternoclavicular joint and medial clavicle. Pectoralis major lies anterior to the vessel, while the clavicle forms the superior bony landmark and the proximal humerus sits lateral and inferior at the glenohumeral joint. Deltoid caps the lateral shoulder, and the scapula provides the posterior framework around the glenoid. Small accompanying veins and regional neurovascular lines are suggested around the axillary region. Emphasis on the clavicular branch matters because the sternoclavicular joint is a common surgical corridor and trauma site, and its blood supply is easy to underestimate when planning incisions or flap elevation along the medial clavicle. During approaches for sternoclavicular joint reconstruction, medial clavicle resection, or drainage of septic arthritis, the short, direct course of this branch creates a predictable bleeding point as it traverses superficial planes near the clavipectoral fascia. Orientation from the anterior also helps correlate surface landmarks used in operative exposure and in angiographic interpretation of axillary and thoracoacromial branching patterns. Bleeding can be brisk.
