Description
Arising from the lumbar sympathetic chain and prevertebral plexuses, fine autonomic fibers descend anterior to the lumbar vertebral bodies and sweep inferolaterally across the pelvic brim to join the testicular (spermatic) plexus. The network then follows the testicular artery toward the deep inguinal ring, continuing distally within the spermatic cord to the testis, with its fibers running medial to the iliac fossa and anterior to the sacroiliac region. Surrounding landmarks include the bony pelvis (ilium, ischium, pubis) and the proximal hip joints at the acetabula. Color contrast typically separates osseous structures from the yellow nerve pathways. Clear orientation matters. This anterior perspective is the one you want when teaching the neural component of the spermatic cord and how visceral pain from the testis can refer to the lower abdomen and groin via sympathetic pathways returning to upper lumbar segments. Surgeons also care about this territory during retroperitoneal lymph node dissection, pelvic tumor resections, and operations near the internal iliac vessels, where traction or thermal spread can disturb autonomic fibers and contribute to chronic orchialgia or altered cremasteric and vascular responses.
