Description
Centered in an anterior abdominal orientation, the gallbladder lies inferior to the liver with its fundus projecting anteroinferiorly, continuing proximally into the neck and cystic duct. A blue cystic vein (often a small venule network from the gallbladder wall) tracks superiorly toward the porta hepatis, running in close relationship to the cystic duct and the cystic artery within the hepatocystic triangle (Calot triangle). Green biliary ducts outline the biliary tree, with the common hepatic duct joining the cystic duct to form the common bile duct, which descends inferiorly toward the duodenum. Red arterial channels accompany the venous drainage. Small vessel anatomy matters here. Surgical relevance is the point of isolating the cystic vein: during laparoscopic cholecystectomy, venous branches from the gallbladder bed and cystic plate can bleed persistently if they are avulsed while the gallbladder is being dissected off the liver. This view also helps teach why establishing the critical view of safety reduces misidentification of ductal structures, since small veins may cross the operative field and obscure the cystic duct and cystic artery. Venous drainage also explains why inflammation in acute cholecystitis can spread to the adjacent hepatic parenchyma via the gallbladder bed. Use this illustration in surgical anatomy modules on the hepatobiliary triangle, in radiology and anatomy teaching that pairs biliary tree diagrams with vascular companions, and in operative technique publications discussing gallbladder bed bleeding control and safe dissection planes.
