Description
Dominating the anterior pelvis, the iliacus spreads across the iliac fossa as a broad, fan-shaped muscle with fibers converging inferiorly toward the iliopsoas tendon. Medially, its belly approaches the pelvic brim and the anterior surface of the sacrum, while laterally it hugs the internal table of the ilium up to the iliac crest. Inferiorly, the muscle narrows as it courses toward the lesser trochanter of the femur, lying deep to the anterior abdominal wall musculature, with rectus abdominis visible superiorly as a midline landmark. Bony references include the iliac crest, anterior superior iliac spine, and the pelvic ring framing the muscle’s origin. Anterior visualization of iliacus matters because it clarifies the iliopsoas compartment and the path the tendon takes beneath the inguinal region toward the hip. This is a practical setup for understanding iliopsoas tendinopathy and internal snapping hip, where the tendon can flick over the iliopectineal eminence or femoral head, and for planning ultrasound-guided iliopsoas bursal injections that target the deep anterior hip.
