Description
Running from the superficial temporal system across the lateral face, the transverse facial artery courses anteriorly over the masseter and along the zygomatic arch region, then continues medially toward the buccal fat pad and the lateral maxilla. Superior to its path sit the zygomaticus major and the infraorbital region, while the parotid duct parallels it closely as the duct advances from the parotid gland toward the buccinator and oral vestibule. Facial expression musculature frames the vessel, with orbicularis oculi superiorly, risorius and buccinator more inferior and medial, and the sternocleidomastoid and carotid sheath structures receding into the cervical background. Branches of the facial nerve (temporal, zygomatic, buccal) and trigeminal sensory branches are positioned in the same superficial plane where the artery is typically encountered. Surgeons care about this artery because it becomes a predictable source of bleeding during preauricular and facelift dissections, parotid approaches, and when elevating cheek flaps over the masseter. Its close relationship to Stensen’s duct makes it a practical landmark, and an easy structure to injure when working in the parotidomasseteric fascia. Small but unforgiving. In trauma and maxillofacial reconstruction, recognizing the transverse facial artery also helps explain cheek hematomas and supports planning for local perforator-based flaps.
