Transverse Facial Artery, Anterior View

Description

Running superficially across the lateral cheek, the transverse facial artery branches from the superficial temporal artery within or just inferior to the parotid region and courses anteriorly, parallel to the inferior border of the zygomatic arch. Its caliber and horizontal trajectory place it superficial to the masseter and near the parotid duct as it approaches the malar region, while the facial artery can be inferred ascending more medially toward the angle of the mouth and lateral nose. Bony landmarks including the zygomatic arch, maxilla, orbital margins, and nasal aperture orient the vessel’s relationship to the midface skeleton. Teeth and the mandibular outline provide additional positional reference. Anterior exposure of the transverse facial artery matters in facial trauma and in procedures that traverse the cheek, because the vessel is a common source of brisk bleeding from lacerations near the zygoma and preauricular region. It is also a practical landmark during parotid surgery and rhytidectomy, where dissection planes near the zygomatic arch risk injury not only to the artery but also to adjacent structures such as the parotid duct and the buccal branches of the facial nerve. Small, superficial, and easy to underestimate. Understanding its anastomoses with the facial and infraorbital arterial systems also clarifies collateral flow patterns relevant to flap planning and post-traumatic hematoma formation.
