Description
Viewed from an inferior aspect, the maxillary second molar is isolated against a semi-transparent maxilla, with the crown oriented superiorly and the root complex projecting inferiorly into the alveolar process. Mesially it approximates the first maxillary molar, while distally it tapers toward the maxillary third molar region, reflecting its typically reduced crown dimensions and a narrower occlusal table. From this underside vantage, the trifurcation region and relative alignment of the mesiobuccal, distobuccal, and palatal roots can be appreciated in relation to the curvature of the dental arch. Inferior visualization matters because it is the clearest way to teach root form and access geometry without occlusal surface distraction. Maxillary second molars commonly present with root fusion or a less divergent root spread than first molars, a pattern that changes endodontic access strategy and increases the risk of missed canals, including a second mesiobuccal canal (MB2) when present. Proximity of the posterior maxillary roots to the floor of the maxillary sinus also makes this tooth clinically relevant in periapical pathology, extractions, and potential oroantral communication.
