Description
Seen from an inferior perspective, the paired ischiocavernosus muscles lie along the medial surface of each ischiopubic ramus, arising near the ischial tuberosity and running anteriorly over the crus of the penis toward the root of the genitalia. Each muscle belly sits lateral to the perineal body and superficial to the urogenital diaphragm region, hugging the pelvic arch as it approaches the pubic symphysis in the midline. The ischium and pubis frame the field, with the inferior pubic rami forming the bony gutter that guides the muscle’s course. Functionally, this is a perineal muscle with a specific mechanical job: it compresses the penile crura and deep dorsal vein region to help maintain penile rigidity during erection, working in concert with bulbospongiosus around the bulb. That relationship matters in pelvic floor examination and in operative planning around the perineum, because scarring, trauma, or iatrogenic injury near the ischiopubic ramus can disrupt the erectile apparatus at the level of the crura.
