Description
Arising from the ischial tuberosity and adjacent ischiopubic ramus, the ischiocavernosus muscle lies superficially along the lateral margin of the urogenital triangle, hugging the inferior pubic ramus as it courses anteromedially onto the crus of the penis. Medial to it, the root of the penis and perineal body occupy the midline, while posteriorly the ischial spine, sacrum, and coccyx frame the pelvic outlet. Deeper and more superior, the levator ani (pubococcygeus and iliococcygeus) forms a muscular sling around the pelvic viscera, separating the perineal compartment from the pelvic cavity. Functionally, ischiocavernosus compresses the crura and deep dorsal venous outflow during erection, so its relationships to the ischium and perineal membrane matter in both sexual medicine and pelvic floor assessment. Injury or denervation in pelvic fracture patterns involving the ischiopubic rami, or iatrogenic trauma during perineal surgery, can disrupt this superficial perineal layer and contribute to erectile dysfunction or perineal pain. This angle also helps teach how the superficial perineal pouch relates to the deeper pelvic diaphragm, a frequent point of confusion when correlating surface anatomy with operative findings.
