Description
Centered in section, the left atrioventricular orifice (ostium atrioventriculare sinistrum) forms a near circular opening between left atrium and left ventricle, bordered by the fibrous mitral annulus. Anteriorly and to the right of the orifice sits the aortic root, with the aortic valve in fibrous continuity with the anterior mitral leaflet (the aorto-mitral curtain). The anterior leaflet spans the anterosuperior margin while the posterior leaflet attaches along the inferoposterior annulus, creating a smaller, scalloped mural component; leaflet free edges face toward the ventricular cavity. Spatial relationships in this cut matter because the mitral valve is not an isolated structure, it is welded into the central fibrous skeleton and immediately adjacent to the left ventricular outflow tract. This is the geometry surgeons and echocardiographers think in when assessing mitral regurgitation from leaflet prolapse, rheumatic commissural fusion with stenosis, or functional regurgitation from annular dilation and papillary muscle displacement. Short axis correlation is straightforward: the orifice area you measure on planimetry aligns with this annular plane, while systolic anterior motion of the anterior leaflet in hypertrophic cardiomyopathy narrows the outflow tract just anterior to it. A critical landmark.
