Description

Oriented from an anterior angle in sectional presentation, the left ventricular inlet (inflow tract) is resolved from the left atrium through the ostium atrioventriculare sinistrum into the left ventricle. The mitral valve leaflets (anterior and posterior) form the atrioventricular junction, with chordae tendineae descending inferiorly to the anterolateral and posteromedial papillary muscles on the ventricular wall. Fibrous continuity between the anterior mitral leaflet and the aortic root is suggested superiorly, while the inflow stream is bounded laterally by the ventricular free wall and medially by the interventricular septum. An anterior sectional view clarifies geometry that is easy to lose in surface renderings: leaflet coaptation, chordal insertions, and the relationship of papillary muscles to the ventricular cavity. This is the corridor traversed in mitral valve repair and replacement, and it is also where annular dilation in functional (secondary) mitral regurgitation alters leaflet tethering and shifts the coaptation line apically. It also maps directly onto echocardiographic reasoning, since the left ventricular inflow tract and mitral apparatus are assessed routinely in parasternal long-axis and apical four-chamber windows when grading regurgitation or stenosis.
