Description
Posteriorly, the adrenal (suprarenal) gland is presented as an encapsulated endocrine organ with a smooth, slightly convex surface that conforms to the superior pole of the kidney. Expect the posterior surface to be shown in close relationship to perirenal fat and the renal fascia, with the gland’s medial aspect oriented toward the great vessels, classically the inferior vena cava on the right and the abdominal aorta on the left. Superiorly, the gland abuts the diaphragmatic crura, while its inferior margin approaches the upper pole of the kidney. The capsule is the dominant external landmark. This posterior perspective matters because it corresponds to how the suprarenal gland is encountered when mobilizing the upper pole of the kidney in retroperitoneal surgery, where the gland can be mistaken for fat or inadvertently violated. Small but consequential venous anatomy sits just medial to this surface, the short right suprarenal vein draining directly into the inferior vena cava and the longer left suprarenal vein draining into the left renal vein, a difference that drives operative strategy and bleeding risk. Orientation is everything. Use this artwork for teaching retroperitoneal relationships in gross anatomy and endocrine blocks, for operative anatomy figures in adrenalectomy and nephrectomy chapters, and for patient-facing education when explaining how adrenal tumors or hemorrhage relate to the kidney and diaphragm.
