Description
Anterior cervical exposure centers on the right and left common carotid arteries (arteriae carotides communes) ascending within the carotid sheaths, medial to the internal jugular veins and anterior to the prevertebral musculature. Inferiorly, the brachiocephalic trunk on the right and the left common carotid artery arise from the aortic arch, then travel superiorly toward the carotid bifurcation at approximately the C4 level near the superior border of the thyroid cartilage. At the root of the neck, the subclavian arteries branch laterally while the common carotids maintain a more vertical, medial course. Clear anterior orientation matters when teaching and planning access to the carotid system, because the common carotid is the proximal conduit for endarterectomy and carotid artery stenting, and its relationship to the internal jugular vein guides safe dissection during an anteromedial sternocleidomastoid approach. The bifurcation zone is where atherosclerotic plaque most often forms, and it sits adjacent to the carotid sinus and carotid body, structures implicated in baroreceptor-mediated bradycardia during surgical manipulation. Small positional errors here have consequences.
