Description

Anterior orientation places the right ventricle in the foreground, forming most of the sternocostal (anterior) surface, while the left ventricle contributes the left cardiac border and the apex, which points inferolaterally to the left. Superiorly, the pulmonary trunk arises anterior to the ascending aorta, and the superior vena cava enters the right atrium on the viewer’s right; the right atrial appendage sits lateral to the aortic root, and the left auricle overlaps the pulmonary trunk on the viewer’s left. The anterior interventricular sulcus descends toward the apex between the ventricles, marking the course of the left anterior descending artery, with the right coronary artery tracking in the right atrioventricular (coronary) sulcus. This frontal external morphology is the working map for correlating surface landmarks with ischemic territories and operative access. The anterior interventricular groove guides identification of the LAD during coronary artery bypass grafting, and the relationship of the pulmonary trunk to the aorta explains why great vessel orientation matters in congenital lesions such as transposition of the great arteries. Clear recognition of the right ventricle as the dominant anterior chamber also supports interpretation of cardiomegaly on chest radiographs and of pericardial effusion effects seen on echocardiography, where the heart’s anterior contour interfaces with the sternum.
