Description
Presented from an anterior (frontal) aspect, the sternocostal surface of the heart is dominated by the right ventricle, with the right atrium forming the right cardiac border and the left ventricle contributing to the left margin and apex. Superiorly, the ascending aorta and pulmonary trunk emerge from the base, with the superior vena cava entering the right atrium on the patient’s right side. The anterior interventricular sulcus descends toward the apex between the ventricles, and the coronary (atrioventricular) sulcus separates atrial tissue from the ventricular mass. Clear orientation. Clinically, this frontal cardiac view matches what you encounter in median sternotomy and what you infer from a standard posteroanterior chest radiograph, where the right heart border is largely right atrium and the left border is shaped by the left ventricle and aortic knob. Appreciating that the right ventricle lies most anterior helps explain why penetrating anterior chest trauma, sternal fractures, or pericardiocentesis trajectories can involve it first, and why anterior ST elevation in left anterior descending artery occlusion localizes to the anterior interventricular territory. Pericardial reflections and great vessel relationships also guide safe cannulation and cross-clamp placement in open cardiac procedures.
