Description

Arising from the right upper lobar bronchus, the apical segmental bronchus (B1) courses superiorly and laterally toward the pulmonary apex to ventilate the apical segment of the right upper lobe. Its origin sits superior to the right pulmonary artery within the hilum, with the segmental branch then tracking toward the pleural dome beneath the cupula and adjacent to the first rib and clavicular region. Peripherally, accompanying segmental pulmonary arteries and veins would be expected to parallel and bracket the bronchial lumen, with veins tending more intersegmental while the bronchus and artery remain centrally segmental. Orientation is keyed to the right lung’s tripartite upper lobe anatomy: B1 lies superior to the posterior segmental bronchus (B2) and typically anteromedial to the anterior segmental bronchus (B3). Segmental anatomy matters when you need to localize disease beyond the lobar level. An apical-predominant opacity, cavitation, or tree-in-bud pattern often narrows the differential toward post-primary tuberculosis or aspiration patterns that favor upper lobe segments, and bronchoscopy reports frequently reference B1 when targeting lavage or biopsy. For thoracic surgeons, clear identification of B1 supports anatomic segmentectomy (S1) for small peripheral malignancies while preserving adjacent segments, and it reduces the risk of unintended devascularization or bronchial misdivision at the right upper lobe. Pulmonology teaching sessions, bronchoscopy simulation curricula, and radiology correlations for segmental localization on CT (including bronchoarterial pairing) all benefit from a clean depiction of the right apical segmental bronchus. This artwork also fits operative atlases and preoperative planning materials where bronchovascular branching patterns guide stapling sequence and margin planning. Anatomical accuracy verified by SciePro's Medical Advisory Board.
