Description
Seen in lateral profile, the facial nerve (cranial nerve VII) emerges from the skull base at the stylomastoid foramen, just posterior to the mandibular ramus and inferior to the external acoustic meatus. From there it courses anterolaterally across the parotid region to form the parotid plexus, where its terminal branches radiate superiorly toward the temporal region and inferiorly toward the mandibular margin and neck. Branching of the trigeminal nerve (cranial nerve V) is also apparent across the face, providing a useful sensory counterpoint to the motor distribution of CN VII. Cervical spinal nerves exit between the cervical vertebrae and descend into the neck. A lateral skull base view is the cleanest way to teach where CN VII becomes surgically exposed: at the stylomastoid foramen and within the parotid gland, where the nerve divides superficial to the retromandibular vein and external carotid artery. This is the anatomic basis for facial weakness after parotidectomy and for the landmark-based dissection used to identify the main trunk near the tragal pointer and posterior belly of the digastric. Bell palsy localizes proximal to the branching pattern, while selective branch injuries create predictable deficits, such as inability to close the eyelids with temporal/zygomatic involvement or an asymmetric smile with buccal or marginal mandibular injury.
