Description

Lateral orbital anatomy is rendered with the globe centered in the bony orbit, the sclera and cornea anteriorly and the optic nerve (CN II) exiting posteriorly toward the orbital apex. The oculomotor nerve (CN III) enters the orbit through the superior orbital fissure and divides into superior and inferior divisions that course anteriorly within the muscle cone, in close relationship to the optic nerve and ophthalmic artery. Superiorly, the CN III superior division tracks toward levator palpebrae superioris and superior rectus, while the inferior division fans to medial rectus, inferior rectus, and inferior oblique, also carrying preganglionic parasympathetic fibers to the ciliary ganglion. Extraocular muscles encircle the globe. Their insertions frame the nerve pathways. This lateral perspective matters because it clarifies the tight compartmental anatomy at the orbital apex, where aneurysm of the posterior communicating artery, cavernous sinus pathology, or trauma can produce a pupil-involving third nerve palsy with ptosis and impaired adduction, elevation, and depression. Spatial relationships to the optic nerve and superior orbital fissure also support teaching of orbital apex syndrome and superior orbital fissure syndrome, where combined deficits of CN III, IV, V1, and VI can be anticipated from the anatomy. Small distances, big consequences.
