Description
A fetus at approximately 20 weeks of gestation lies in a curved, flexed posture within the translucent amniotic sac, seen from the left side. The large domed cranium dominates body proportions, with closed fused eyelids, a formed nasal tip, and a well-modelled ear auricle set low on the head. One upper limb reaches upward with separated fingers, while the lower limbs are drawn toward the abdomen with defined knees, ankles and feet. The umbilical cord emerges from the abdominal wall and coils through the amniotic fluid toward the discoid placenta seen behind and above the fetus, its chorionic surface applied to the vascular uterine wall; fine particulate vernix and cells are suspended in the surrounding fluid. Twenty weeks marks the midpoint of pregnancy and the window of the standard second-trimester anomaly scan, when biometry such as biparietal diameter, head and abdominal circumference and femur length are measured, cardiac outflow tracts and the spine are surveyed, and placental site and cord insertion are documented. Amniotic fluid volume assessed at this stage informs suspicion of renal or swallowing anomalies, while cord coiling and vessel number are noted routinely. Maternal perception of fetal movement typically begins around this period, and the flexed limb posture shown reflects the muscle tone developing at this gestation.
