Description
The fetus lies curled in the amniotic cavity with the head turned to the right of frame and the trunk flexed, knees drawn toward the chest in the characteristic fetal attitude of the late second trimester. The umbilical cord passes from the anterior abdominal wall at the umbilicus toward the placenta, its spiral of two umbilical arteries around the single umbilical vein clearly visible beneath the amnion-covered surface. Behind the fetus the placental disc is seen from its fetal surface, with chorionic plate vessels branching across it before converging at the cord insertion. Fine surface detail includes the fetal ear, the small fingers of the hand near the cord, the toes, and early scalp hair, while the surrounding uterine wall forms the soft outer boundary of the scene. At 23 weeks the fetus sits at the threshold of viability, making this stage central to counselling on extreme preterm birth, antenatal corticosteroid timing and neonatal resuscitation planning.
