Description

A fetus at approximately 31 weeks of gestation lies curled within the amniotic cavity, the flexed limbs drawn toward the trunk in the characteristic fetal attitude, with the head turned so that the ear, closed eyelids and a full covering of scalp hair are visible. Subcutaneous fat has begun to smooth the contours of the cheeks, hands and feet, and the fingers and toes are fully formed with distinct nails. The umbilical cord spirals from the anterior abdominal wall, its paired umbilical arteries and single umbilical vein indicated by the twisted red and violet vessels, and inserts near the centre of the disc-shaped placenta. The maternal surface and chorionic vessels of the placenta are seen against the vascular uterine wall enclosing the fluid-filled cavity. Week 31 falls in the late preterm to moderately preterm window, when lung maturation, surfactant production and rapid fat and brain growth dominate development, and when survival with neonatal intensive care is high. Visualising cord coiling, cord length and placental insertion supports discussion of nuchal cord, true knots, cord compression and placental insufficiency, all of which are assessed on third-trimester ultrasound and Doppler studies of umbilical artery flow. Fetal lie and presentation at this stage also inform counselling about version, breech presentation and mode of delivery.
