Description
A fetus of approximately 27 weeks gestation lies in a flexed, curled attitude within the fluid-filled amniotic sac, seen from the left side with the head to the upper right. The cranial vault is large relative to the trunk, the scalp carries fine hair, and the external ear (auricle) is fully modelled with a defined helix and lobule. Both hands are brought up in front of the mouth with the fingers overlapping, while the lower limbs are strongly flexed at hip and knee so that the feet reach toward the face. The translucent amniotic membrane and surrounding amniotic fluid separate the fetal skin from the vascular, folded uterine wall, and subcutaneous fat is beginning to smooth the contours of the shoulders, abdomen and buttocks. Week 27 marks the close of the second trimester and the beginning of a period of rapid weight gain and neurological maturation. At this stage the eyelids have opened, the fetus responds to sound and light, and pulmonary development has advanced to the point where surfactant production and alveolar sac formation greatly improve survival with neonatal intensive care. The image is useful when discussing extreme preterm birth, respiratory distress syndrome, fetal biometry and estimated fetal weight on third-trimester ultrasound, as well as fetal presentation and the flexed posture assessed during obstetric examination.
