Description
At approximately 32 weeks of gestation the fetus fills much of the uterine cavity, shown here in a flexed attitude with the spine curved, hips and knees drawn up, and the plantar surfaces of both feet visible near the anterior abdominal wall. The head is large relative to the trunk, with fused eyelids, a formed external ear with defined helix and antihelix, patterned scalp hair, and one hand raised so the fingers lie across the cheek. Subcutaneous fat has begun to smooth the contours of the limbs and trunk, and the skin appears less translucent than in earlier trimesters. The translucent amniotic membrane encloses the fetus with amniotic fluid between it and the body, while the vascularised uterine wall and endometrial lining form the outer chamber. The third trimester is the period of rapid weight gain, brown fat deposition and pulmonary surfactant maturation, so imagery of this stage supports counselling on late preterm birth and neonatal viability. Fetal lie, presentation and degree of flexion at this point inform obstetric assessment by palpation and ultrasound, and guide discussion of breech presentation or external cephalic version toward term. Amniotic fluid volume, appreciable here as the space between the fetal surface and the membrane, is a routine surveillance parameter, with oligohydramnios and polyhydramnios both prompting closer monitoring. Purposeful hand-to-face movement reflects the neuromuscular coordination that underlies observed fetal behavioural states.
