Description
Seen from the side, the maternal torso is rendered semi-transparent so the gravid uterus fills the abdomen from the pelvic brim to well above the umbilicus. Within the amniotic sac a fetus of about 35 weeks lies in cephalic presentation, the vertex directed toward the pelvic inlet, spine curved along the maternal anterior wall, and the limbs flexed against the trunk in the typical fetal attitude. The umbilical cord loops from the fetal abdomen toward the placenta on the uterine wall, while the maternal intestines are displaced superiorly and posteriorly. The cervix, urinary bladder anteriorly, pubic bone and sacrum outline the bony and visceral boundaries of the pelvis in this projection. By week 35 the fetus has largely completed organogenesis and is gaining subcutaneous fat and muscle bulk, with lung maturation continuing through surfactant production. A lateral view of this stage is the standard way to explain lie, presentation and engagement, and it clarifies why fundal height, symphysis-fundal measurement and Leopold manoeuvres give reliable clinical information late in the third trimester. The same anatomy underlies common late-pregnancy complaints: bladder compression and urinary frequency, gastric reflux from upward displacement of the stomach and bowel, and lumbosacral strain from the shifted centre of gravity. It also frames discussion of cervical assessment and the descent of the presenting part before labour.
