Description
A fetus of about 35 weeks gestation lies in a flexed, head-down attitude within the amniotic cavity, seen in left lateral profile. The rounded cranium carries fine scalp hair, the eyelids remain closed, and the external ear, nose and lips are fully modelled, with subcutaneous fat smoothing the cheeks and limbs. Both upper limbs are drawn across the chest with the hands near the face, while the lower limbs are folded tightly against the abdomen in the classic fetal posture. The translucent amniotic sac membrane surrounds the fetus, suspended in amniotic fluid, and the umbilical cord with its coiled arteries and vein passes toward the vascular placental surface on the inner uterine wall. At 35 weeks the fetus occupies most of the uterine cavity, and the amount of free amniotic fluid relative to fetal mass is visibly reduced, which is why late third-trimester ultrasound emphasises amniotic fluid index, fetal lie and presentation. This is the period of accelerating weight gain, brown and white fat deposition, and continuing lung maturation, so infants born now are late preterm rather than term and remain at risk of transient tachypnoea, feeding difficulty and thermoregulatory instability. The cord position, placental site and degree of flexion depicted are the same features assessed when planning external cephalic version, monitoring growth restriction, or counselling on timing of delivery.
