Description
Posterior anatomy of the male large intestine is presented with emphasis on the plicae semilunares coli, the semilunar folds created where the circular muscle and mucosa infold between adjacent haustra. These folds arc transversely across the colic lumen, lying deep to the teniae coli and giving the colon its half moon internal contour. Along the lateral flexures, the folds align with the segmental sacculations, superior toward the splenic flexure and inferior toward the sigmoid and rectal transition, with the folds becoming less conspicuous as the bowel approaches the terminal rectum. Orientation from the posterior matters because it mirrors how the colon and rectum are approached during mobilization and pelvic dissection, when surgeons must keep the bowel wall, mesocolon, and peritoneal reflections in mind to avoid full thickness injury. Semilunar folds are also a practical landmark in endoscopy and pathology: they delineate haustral compartments where fecal stasis, diverticulosis (classically between teniae in the sigmoid), and mucosal edema in colitis can alter the usual fold pattern. Loss of normal haustration and effacement of folds is a recognized macroscopic cue in severe ulcerative colitis and toxic megacolon. A subtle warning sign.
