Description
Seen in lateral profile, the distal sigmoid colon transitions into the rectum at the rectosigmoid junction, with the plicae semilunares coli forming evenly spaced crescentic mucosal folds along the bowel wall. These semilunar folds lie on the luminal aspect and align with the outer contour created by the haustra, producing a subtly scalloped silhouette along the antimesenteric border. Inferiorly, the rectum appears smoother as the typical haustral segmentation and semilunar pattern diminish approaching the pelvic segment. Orientation is side-on. A lateral study of the semilunar folds is useful when teaching why the colon does not behave like a uniform tube: haustral compartmentalization and the plicae semilunares influence how gas and fecal material segment and move during peristalsis. Endoscopically, the folds can mimic or obscure small sessile lesions, and understanding their normal spacing and curvature helps reduce false positives during colonoscopy in the distal sigmoid. Surgical trainees also benefit from appreciating the characteristic lateral bend at the rectosigmoid junction, a frequent site of volvulus and a key transition during sigmoid colectomy and low anterior resection planning.
