Description
Posteriorly oriented, lateral spatial context places the semilunar folds (plicae semilunares coli) along the wall of the large intestine as it approaches the hepatic flexure, where the ascending colon turns sharply to become the transverse colon. The haustra are implied as sacculations between adjacent plicae, with the teniae coli running longitudinally and creating the segmented contour. Superior to the flexure sits the inferior surface of the liver, while the right colic (hepatic) flexure lies inferior to the costal margin and lateral to the duodenum and head of the pancreas. A tight bend. Understanding where the semilunar folds sit relative to the hepatic flexure matters when you are teaching why the colon does not behave like a smooth tube, and when you are interpreting luminal contour on contrast studies or colonoscopy. The flexure is a common site of difficult scope passage and can be a locus for stool stasis, angulation, and pain during insufflation, and the haustral pattern can be effaced in colitis or altered by an obstructing carcinoma proximal to the bend. Posterior relationships also frame surgical mobilization, where right colon mobilization proceeds along Toldt’s fascia to protect the retroperitoneal duodenum, ureter, and gonadal vessels.
