Description
Posterior anatomy of the male sigmoid colon is centered on the taenia coli pattern, with emphasis on the taenia mesocolica (mesocolic band) where the sigmoid mesocolon attaches along the posterolateral aspect of the bowel wall. From this orientation, the sigmoid loop occupies the pelvic cavity inferior to the pelvic brim, curving toward the rectosigmoid junction as it approaches the midline. Haustra are implied between the longitudinal bands, while the mesenteric reflection marks the transition from peritonealized colon to the posterior abdominal and pelvic wall. Clean landmarks. Focusing on the taenia mesocolica matters because it marks the line of mesenteric attachment and, by extension, the corridor for the sigmoid branches of the inferior mesenteric artery and their accompanying veins and lymphatics within the mesosigmoid. During sigmoid colectomy or rectosigmoid resection, surgeons often orient themselves by this posterior mesenteric strip to select a safe plane for mobilization while preserving the hypogastric nerves and avoiding ureteral injury on the posterior pelvic sidewall. The posterior view also supports teaching of sigmoid volvulus, where a long, redundant mesosigmoid creates a narrow root that predisposes the loop to twist, producing closed-loop obstruction and ischemia.
