Description
Viewed from the left lateral aspect in an adult male, the distal transverse colon sweeps into the splenic flexure beneath the left hemidiaphragm, with the descending colon continuing inferiorly along the posterior abdominal wall. The sigmoid colon (colon sigmoideum) occupies the left lower quadrant and pelvis, curving anteriorly and medially as it approaches the rectosigmoid junction, while remaining tethered posteriorly by its mesocolon. Superior to this bend sit the spleen and tail of the pancreas near the splenic hilum, and posteriorly the flexure lies close to the left kidney and the quadratus lumborum. Spatial relationships are the point. Teaching the splenic flexure as a high, posterior colonic turn matters because it anchors the left colic gutter and marks a common site of sharp angulation during colonoscopy, where loop formation can increase patient discomfort and raise perforation risk. This lateral orientation also helps when discussing watershed ischemia at Griffiths point near the flexure, where marginal artery perfusion may be tenuous in hypotension or after ligation during left-sided colectomy.
