Description
Curving through the left lower abdomen into the pelvis, the terminal sigmoid colon (colon sigmoideum) is shown with its mesocolon in lateral profile, emphasizing the S shaped course as it descends inferiorly toward the rectosigmoid junction. A peritoneal fold suspends the sigmoid segment posteriorly, with the mesosigmoid attaching along the posterior pelvic wall while allowing the bowel to remain mobile and loop anteriorly. Proximally, the lumen transitions from descending colon to sigmoid, and distally the bowel straightens as it approaches the pelvic midline. Mesenteric tissue dominates the view. Lateral depiction of the mesosigmoid matters because it clarifies why the sigmoid colon can swing into different pelvic positions, a frequent source of variability during colonoscopy and cross sectional imaging. Redundancy and a long mesocolon predispose to sigmoid volvulus, where the bowel twists around its mesenteric axis and can compromise branches of the inferior mesenteric artery supplying the segment. Surgeons also judge this mobility when planning sigmoid colectomy for diverticular disease, balancing reach for a tension free anastomosis against the need to preserve vascular pedicles.
