Viewed from the lateral aspect in an adult male, the distal large intestine is traced from the left colic (splenic) flexure through the descending colon into the sigmoid colon (colon sigmoideum) as it curves inferiorly toward the pelvis. Superiorly, the transverse colon transitions at the flexure under the left hemidiaphragm, adjacent to the splenic region, before the descending colon runs caudally along the left posterior abdominal wall. The sigmoid segment then assumes its characteristic S shaped course, shifting anteriorly and medially as it approaches the rectosigmoid junction. Peritoneal reflections and mesocolon attachments are implied by the change in mobility between the retroperitoneal descending colon and the more mobile pelvic sigmoid. A lateral evaluation clarifies where the splenic flexure sits in relation to the left kidney and the costal margin, and why this region can be a fixed point during colonoscopy.
