Description
Centered on the superior extremity of an adult lung, the apical parenchyma forms a smooth dome that rises above the level of the clavicle and first rib, tapering inferiorly into the upper lobe. Medially, the apex relates to the mediastinal pleura, while its lateral surface remains continuous with the costal pleura along the thoracic inlet. Superior and posterior to this region lie the cupula and adjacent neurovascular structures at the root of the neck, including the subclavian vessels and the lower trunks of the brachial plexus. Apex localization matters because small shifts at the thoracic inlet have outsized clinical consequences. Apical blebs and bullae are a common source of primary spontaneous pneumothorax, and their proximity to the pleural dome explains why even limited air leakage can produce rapid pleural separation at the top of the hemithorax. It also frames classic teaching around Pancoast (superior sulcus) tumors, where apical masses can invade the parietal pleura and adjacent tissues to produce shoulder pain, ulnar-distribution symptoms, or Horner syndrome.
