Description
Superior to the first rib, the pulmonary apex (cupula pulmonis) rises into the root of the neck, projecting above the medial third of the clavicle and the costal cartilage of rib 1. Laterally, its pleural covering forms the cervical pleura, while medially it approaches the superior mediastinal boundary. Inferiorly, the apex transitions into the upper lobe parenchyma, with the costal surface curving against the inner thoracic wall. Small, high-stakes anatomy. Clinically, the pulmonary apex is where thoracic and cervical anatomy collide, and that matters whenever air finds the pleural space. A needle placed too medially in an infraclavicular approach for subclavian venous access can puncture the cervical pleura and the underlying lung apex, producing an iatrogenic pneumothorax that often first collects superiorly. Apical pathology also has a distinct signature: Pancoast (superior sulcus) tumors at the lung apex can invade adjacent structures in the thoracic inlet, and apical blebs are a common substrate for spontaneous pneumothorax in young patients.
