Seen from the side with the maternal abdominal wall rendered semi-transparent, the gravid uterus occupies the lower abdomen and rises above the pubic symphysis toward the umbilical level. Within the amniotic sac, a fetus of about 17 weeks lies in a flexed attitude, head directed anteroinferiorly, with the umbilical cord looping from the ventral abdominal wall to the placental disc applied to the uterine wall. Posterior to the uterus the rectum descends toward the pelvic floor, while the urinary bladder sits anteriorly behind the pubis; loops of small intestine are displaced superiorly and posteriorly by the enlarging fundus. The lumbar vertebrae, sacrum and hip bone provide the skeletal frame, and the woman's hands rest on the lower abdomen and lumbar region. A mid-second-trimester lateral projection makes the mechanical consequences of uterine growth explicit: cranial displacement of bowel, anterior traction on the abdominal wall, and increased lumbar lordosis that underlies common low back and pelvic girdle pain. The relationship of the fundus to the umbilicus is the anatomical basis of symphysis-fundal height measurement at antenatal visits, and the anterior position of the bladder explains urinary frequency and reduced functional capacity.
