Description
Curving across the upper abdomen, the transverse colon (colon transversum) arches from the right colic (hepatic) flexure to the left colic (splenic) flexure, forming its primary curvature beneath the greater omentum. Its convex border lies anterior and inferior, while the concave border is directed superiorly toward the transverse mesocolon (mesocolon transversum) and posterior abdominal wall. Lateral relationships sit with the right and left colic flexures near the liver and spleen, while the midportion typically hangs more inferiorly in the midsection, variable with body habitus and mesocolic length. This configuration matters because the transverse colon is the most mobile segment of the large intestine, suspended by the transverse mesocolon and draped by the great omentum, and its position dictates both symptom localization and operative exposure. Volvulus of the transverse colon, though less common than sigmoid volvulus, depends on redundancy and mesocolic laxity, and the primary curvature helps explain the loop that can twist around its mesentery. During colectomy, awareness of the transverse mesocolon’s attachment and the proximity of the middle colic vessels and pancreatic inferior border guides safe entry into the lesser sac and helps avoid injury during mobilization.
