Description
Sectional anatomy through the male transverse colon places the colon transversum high in the abdominal midsection, suspended from the posterior abdominal wall by the mesocolon transversum. Expect the colonic lumen with mucosal folds and a surrounding muscular wall, with the greater omentum draping anteriorly and the transverse mesocolic vessels running centrally within the mesentery. Superior relations commonly include the inferior surface of the liver on the right and the greater curvature of the stomach toward the left, while posteriorly the pancreas and secondarily retroperitoneal structures sit deep to the mesocolic attachment. Spatially, the transverse colon spans right to left, with the hepatic flexure lateral and superior to the ascending colon and the splenic flexure high and lateral near the spleen. This sectional perspective matters because the transverse mesocolon defines a key surgical and embryologic boundary between supramesocolic and inframesocolic compartments. Mobilization of the transverse colon during colectomy or exposure of the pancreas depends on understanding the plane between the transverse mesocolon and the anterior surface of the pancreas, where injury risks include the middle colic vessels and adjacent gastroepiploic arcade. Ischemia and anastomotic complications often trace back to vascular territory, so seeing how the mesocolic pedicle relates to the bowel wall is not academic.
