Description
Arising along the medial border of the foot, the abductor digiti minimi is presented as a superficial muscle of the first layer of the plantar compartment, running anteriorly from the calcaneal tuberosity and plantar aponeurosis toward the base of the proximal phalanx of the fifth toe. Its belly lies lateral to flexor digitorum brevis and inferior to the calcaneus, with the long plantar ligament and the cuboid positioned deep and slightly dorsal. More distally, the muscle parallels the lateral longitudinal arch, adjacent to the fifth metatarsal and the metatarsophalangeal joint of digit V. Clean landmarks. Clinically, this view helps clarify the lateral plantar nerve and artery course as they pass anteriorly between the first-layer muscles, a relationship that matters in heel pain workups and plantar surgical exposure. Hypertrophy or spasm of abductor digiti minimi can contribute to lateral plantar foot pain and has been discussed in runners with altered lateral column loading, while entrapment of the first branch of the lateral plantar nerve (Baxter nerve) commonly produces medial calcaneal pain that can be confused with plantar fasciitis.
