Description
Positioned at the superior end of the lumbar column, the L1 vertebra is highlighted in blue above L2 through L5, with the intervening intervertebral discs rendered in white. The large vertebral body sits anterior to the vertebral arch, while paired pedicles project posteriorly to meet the laminae and complete the vertebral foramen. Laterally, the transverse processes extend from the junction of pedicle and lamina, and the spinous process projects posteriorly from the midline. Five lumbar levels are visible. Orientation at L1 matters because it marks the transition from thoracic rigidity to lumbar mobility, and it sits adjacent to landmarks clinicians use every day, including the thoracolumbar junction and the conus medullaris (typically around L1 to L2 in adults). Fractures at this level often follow axial loading (burst fracture) or flexion distraction mechanisms (Chance fracture), and the relationship between the vertebral body, pedicles, and canal helps explain retropulsed fragments and potential cauda equina compromise. Pedicle anatomy at L1 also guides screw trajectory in posterior fixation, where a few degrees of medial angulation can be the difference between solid purchase and canal breach. A useful level to teach. Use this asset in gross anatomy and musculoskeletal courses to reinforce lumbar vertebra morphology, or in spine surgery and radiology publications when labeling L1 for thoracolumbar localization and instrumentation planning.
