Description
Centered on the lumbar segment of the vertebral column, the first lumbar vertebra (L1) sits inferior to T12 and superior to L2, forming the upper part of the lower back just cranial to the lumbosacral junction. A stout vertebral body lies anteriorly, while the vertebral arch extends posteriorly via paired pedicles and laminae to the spinous process; laterally, the transverse processes project toward the paraspinal musculature. Superior and inferior articular processes frame the zygapophyseal (facet) joints, and the intervertebral discs occupy the spaces between adjacent vertebral bodies. The sacrum and coccyx appear inferior in the same column, establishing the transition from mobile lumbar spine to fused sacral segments. L1 matters because it is a frequent landmark at the thoracolumbar junction, where biomechanics shift from the rib-bearing thoracic spine to the load-bearing lumbar spine. Compression fractures often cluster in this region after axial loading, and the morphology of the pedicles and lamina informs posterior fixation when short-segment instrumentation is required.
