Description
Framed from an anterior perspective, the first lumbar vertebra (L1) appears at the superior end of the lumbar series, with the vertebral body centered in the midline and adjacent intervertebral discs separating it from T12 above and L2 below. The L1 body is broad and weight bearing but reads slightly smaller than the more caudal lumbar bodies, while the pedicles and transverse processes project posterolaterally from the body margins. Inferiorly, the sequence continues through the lower lumbar vertebrae to the sacrum, whose fused segments form a tapered base in the same frontal alignment. Intervertebral discs are rendered as distinct fibrocartilaginous pads between each vertebral level. An anterior lumbar view is the cleanest way to teach vertebral body morphology and disc spacing without the visual noise of the posterior elements, and it mirrors the corridor approached in anterior lumbar interbody fusion (ALIF) when the surgeon works directly onto the disc and vertebral body. L1 also sits at the thoracolumbar junction, a common site for compression fractures, and its relationship to the T12 L1 disc helps explain why junctional mechanics differ from L4 to L5. Body size and shape at L1 matter when discussing vertebral body fractures, endplate changes, and anterior osteophytes seen in degenerative disc disease.
