Seen in right lateral view, the sixth ribs are highlighted along the mid-thoracic cage as they sweep from their posterior articulations with the thoracic vertebrae toward the anterior costal cartilages. Each rib originates at the costovertebral and costotransverse joints, then curves anteroinferiorly, with its head and neck lying medial and posterior to the angle of the rib and its shaft. Along the inferior border of the bony shaft, the costal groove (sulcus costae) forms a protected channel for the intercostal neurovascular bundle, positioned between the internal and innermost intercostal muscle layers. Thoracic spinous processes and intervertebral discs provide midline orientation posterior to the ribs. Level-specific rib anatomy matters when you are counting spaces and planning access to the pleural cavity, because the 6th rib and its intercostal space commonly serve as surface landmarks for thoracostomy tubes, thoracoscopy portals, and targeted intercostal nerve blocks. The costal groove explains why clinicians place needles and chest drains just superior to the upper border of a rib to avoid the posterior intercostal artery, vein, and nerve that run in the groove, and it also frames typical fracture pain patterns when the bundle is contused.
