Description
Running along the medial aspect of a male upper limb, the ulnar nerve descends from the distal arm to the wrist, passing posterior to the medial epicondyle at the elbow before entering the forearm between the heads of flexor carpi ulnaris. Distally, it courses deep to flexor carpi ulnaris on the ulnar side of the forearm, traveling with the ulnar artery and accompanying veins toward the ulnar (medial) border of the hand. Surrounding musculature is rendered in situ, with the biceps brachii more anterior, the flexor-pronator mass occupying the anteromedial forearm, and tendinous structures converging toward the carpus and digits. The medial perspective keeps the ulna and its soft tissue relationships as the dominant landmarks. Clinically, this trajectory frames the two most interrogated regions of ulnar neuropathy: the cubital tunnel at the elbow and Guyon’s canal at the wrist. That posterior-to-medial-epicondyle segment explains the classic tingling elicited by percussion, and it is also where valgus stress, osteophytes, or prolonged elbow flexion can provoke conduction slowing in electrodiagnostic testing. Following the nerve alongside the ulnar vessels in the distal forearm supports surgical planning for ulnar nerve decompression, medial epicondylar approaches, and safe dissection planes when addressing forearm trauma or vascular access.
